Breast density notification increases levels of confusion and anxiousness among women

Breast density notification is being rolled out across Australia, but evidence suggests that women do not feel more informed by it.

New research by experts at the 爆料王 shows that breast density notification聽is聽leaving聽some聽women confused聽and anxious聽about their聽breast聽health.

The聽notification聽program聽is聽designed to聽advise聽women that聽their breast聽density is a risk for developing cancer, as well as highlighting聽that dense breasts could prevent cancer from being聽observed聽on a mammogram.聽

Breast聽density notification is聽currently聽being rolled out across Australia,聽and is already mandatory in the US,聽but until now there has been no聽high-quality聽research into the impact of these notifications on women.

Lead author聽of the聽听蝉迟耻诲测,听Dr Brooke Nickel聽from the聽School of Public Health, said:聽鈥淏reast density聽notification聽is based on聽the premise that giving聽women聽information will empower them聽to聽make individual choices about their health.

鈥淚n theory this is a聽really聽good聽thing聽but what we found in practice was that breast density notification made women more confused and anxious about their聽breast聽health.聽Women also did not feel more informed.鈥

Estimates suggest that between 25 and 40 percent聽of聽women have denser breast tissue which can increase the risk of developing cancer and make it harder for cancers to be detected聽through a聽mammogram.聽

For women with denser breasts,聽further聽screening via ultrasound, MRI and contrast-enhanced mammography has been shown to detect cancers. However, most of these聽additional聽screening methods聽mean out-of-pocket costs for women, and聽the long-term benefits in terms聽of mortality rates, as well as聽unintended adverse聽consequences,聽such as widening health inequality,聽are not well-evidenced.聽

Carrying out the research

To understand the impact of breast density notification, the researchers聽carried out a randomised control trial聽involving 2401 women聽across 13聽BreastScreen聽sites in Queensland聽between September 2023 and July 2024.聽

As part of the聽randomised聽controlled聽trial, women identified as having dense breasts聽received one of three聽outcomes:

  1. a notification of breast density in their screening results letter plus a聽leaflet with聽additional聽information about聽having denser聽breasts;
  2. notification of breast density in the results letter plus a link to online video-based health literacy-sensitive information;聽
  3. no notification of breast density in their results letter聽(control group).

Significantly,聽11.5 percent and 9 percent of the聽two聽intervention groups respectively reported聽feeling confused聽after聽receiving a breast density notification聽alongside a normal screening result, compared to only 2.7 percent of women in the control group.聽

Levels of聽anxiousness聽among the participants聽about what to do聽regarding聽their breast health聽were also higher in the intervention groups, rising to 20.8 percent聽(outcome 1)聽and 20.5 percent聽(outcome 2)聽respectively, compared to the 18 percent of women in the control group.聽

Both notified groups also had significantly higher intention聽to talk to their GP about their screening results聽鈥撀22.8聽percent and聽19.4聽percent respectively聽鈥撀燾ompared with聽only 12.9 percent in the control group.聽

Dr Nickel聽said: 鈥淐reating confusion聽and worry聽for women about their breast health is not a good outcome. And while it might seem positive that notified women want to seek further advice from their GP, there聽is聽currently聽limited聽evidence-based聽clinical pathways that GPs can offer, with most of the聽suggested recommendations聽leaving women with out-of-pocket costs.

"Internationally,聽we聽encourage聽governments to聽consider gathering聽additional data on beneficial and equitable clinical pathways and聽to聽plan the provision of breast density information聽carefully聽to ensure that聽the聽benefits聽of breast density notification聽outweigh potential harms聽for all women.鈥
